I. Church/Christian Organization Use ¡V Annual Fee
A. Annual Fee: US$65.00, accept check, bank draft, and credit card payment.
B. Effective period: Within one year from the date of receiving the form and the annual fee. Renewal notification will be sent one month prior to expiration.
C. Usage: Church worship, fellowship/small group meetings, church retreats and other church meetings
D. Songs allowed to use and method:
1. All songs in New Heart Singalong Books which are originally copyrighted and owned by New Heart. Songs which are not originally owned by New Heart, please contact the original owners of the songs for permission to use.
2. Lyrics and music appear in New Heart Singalong Books can be photocopied on church bulletin, small group songbook and retreat handbook.
3. Songs appeared in ¡§Let¡¦s Worship¡¨ Singalong Books and Lyrics Book.
4. 100 Favorite Hymns "Hymns of my Heart" - Please contact us if the quantity of photocopy from this Singalong Book exceeds 15 copies.
E. No limitation on total no. of songs and quantity of the printed copy
F. For reproduction of the music to simplified music score or translation of the lyrics to other languages, please contact New Heart for prior approval. On all reproduction, please include name of the original composers, lyricists; please also include New Heart copyright information.
G. The following products and music cannot be reproduced, please contact local Christian bookstores or New Heart to purchase the original products:
1. CD, Tape
2. Sound Track
3. Choir Book ¡V If photocopy is necessary, the photo-copyright fee is US $1.00 per song per copy
4. Octavo ¡V Each New Heart song is available in Octavo, they are US $2.00 each

II. Church/Christian Organization Use ¡V Non-annual Fee
A. Only limited to use of New Heart Singalong Books. Please do not reproduce songs which are not originally owned by New Heart, please contact the original owners of the songs for permission to use.
B. For rehearsal or meeting use, photocopy of each song is US$0.50 per copy. Please mark ¡§For rehearsal only¡¨ on each photocopy.
C. Please keep your own record of songs being used and the number of the photocopy, and please send in the record along with the copyright fee every 6 months.
D. Overhead master of the respective song in the Singalong Books can be photocopied

A. Copyright Fee: US$30.00
B. No limitation on total number of songs and quantity of the printed copy
